Is it compulsory to fill up Annex A Form when applying PR??

Any advice??
If employer refuse to fill up then how?
Payslip is the way to prove that you are currently employ in the company?

Payvoucher/slip and EP. What more do you need? No, Annex A is not really required. It's nice but not necessary in the case of an obstinate employer.
