How can I change my babies name?

My baby is 8 months old and we need to change his name on the birth certificate.
I have looked on the ICA website and can find no direction as to how to do this. I would be so grateful to know.....;
1. Can I still change his original name on his birth cert?
2. Do you know where I go? (if ICA then where, which floor etc)
3. What forms I need to fill out?
4. What is the maximum age he can be before his original name needs to become an (aka)?

Any advice or direction would be much appreciated

Q) Can I change my child's name in the Birth Certificate?

A) A request for an alteration of the child's name in the birth register can be accepted upon payment of the prescribed fees and before the child is one year old. This is provided that no certified extract of the birth register has been issued during this period. This alteration can only be done at Registry of Births at ICA Building. A birth extract, not a new birth certificate, will be issued reflecting the change of name.
