Part C of the form, height n weight, do we have to measure our own or measure on the day itself? cos i think they need to take our measurement on the day itself.

Part D, it stated eng, maths & sci(indicate all subjects), mine is a combine sic so, should put phy/chem in the box or on the next column where they put subject and grade again?

as for the wt and ht part of the form, i wrote in my last known ht and wt. If it is wrong, the lady taking ur stats will write the correct one in for you. at least that's wat she did for my ht. she minus 1 cm off! bah!

for part D- i put mine as SCIENCE (PHY/CHEM) and the grade in one box, not separate. as like you, mine is a combined subject.
