Urgently looking for a gynae who can treat PCOS

Hi there,
I am diagnosed with PCOS and High BP. I am unable to concieve. Has anyone been to a gynae who was able to treat you with PCOS and help you concieve naturally without IUI or IVF????
I am ok with IUI...but my gynae is pressurising me for IUI after 3 rounds of clomid. I am just 26 yrs now.......and i think I have chance of concieving naturally.
Please be kind and recommend a good doctor, preferably an Indian gynae. Please share the contact details of the gynae.

If you're only 26 and not pregnant in a few months of using Metformin & Clomid, it'll be best to see a reproductive endocrinologist (fertility doc) because PCOS might not be your only issue.

A fertility doctor can run blood tests for lots of things and also order a hysterosalpingogram (HSG), which checks for fallopian tube blockage and checks the shape of the uterine cavity and check for polyps/fibroids. Oftentimes the pressure from the HSG will help clear the tubes if they're filled with debris. Another test is a water ultrasound, also known as a sonohysterogram. Neither test is fun.

They also might want to run tests on your husband/partner because you might not be the only one with an issue. He won't like this, but he'll get over it.

But even if you're fine every other way and just want to use clomid/Metformin combo, a fertility doc will be more experienced monitoring ovarian follicle growth through pelvic ultrasound and giving you a shot to force ovulation once the follicles reach a certain size, because (as you know) women with PCOS sometimes don't ovulate...and those ovulation predictor strips from the drug store even mention on the instructions that they're not for women with PCOS.

Another thing, Clomid doesn't work for all women (esp if they're older) and other times it works too well (esp if they're younger)...another reason to be monitored closely....so you don't ovulate 10 eggs and end up with 5 being fertilized and implanting.

I'm sorry you're going through this. I don't have a suggestion for a local doctor because I dealt with this before moving here...finally had a baby in my late 30s last year with the help of modern science. Just don't get too hung up at the gynecologist for too long and don't discount IUI or IVF because sometimes it's necessary....Doesn't matter how your children get here as long as they're here.

BedokAmerican - that was very well put. And I would agree with you on all counts. For PCOS, my personal advice would be to begin the treatment as early as possible. I delayed my treatment thinking I had enough time and now three years have passed and we are still running the rounds:(

On a general note - I have found a doctor in NUH who is a specialist in infertility issues and especially PCOS related matters- Dr. Anupriya Agarwal.
Has anyone been treated under her? - Any reviews would be very welcome.

My own experience here.

I have PCOS since 2006. Got married in 2008 and had been struggling to conceive. I saw Dr. Selina Chua in Mt. E from Oct 2010 onwards. She put me on Clomid (200 mg) for 5 cycles. On the 5th cycle i had a HSG test as well, to check if my fallopian tubes were clear, and thankfully they were.
I conceived on my 5th cycle and now have a precious 1.5 yr old.

I highly recommend Dr.Chua. If you're looking for somebody who'll hold your hand through this and be empathetic, then she's not the doctor for you. She is very professional, and tells you like it is. She was calm, cool and collected when I had a pregnancy complication at 33 weeks (preeclampsia) and delivered my baby prematurely, saved both me & my child.

And this is just my personal opinion to OP - please dont try to limit yourself to an Indian doctor. I am an Indian as well, but there are plenty of skilled non-Indian doctors with vast expertise in fertility related problems.. and you'll just be depriving yourself if you limit yourself.

Hi All,

Just wanted to understand how does the medical system really works here.
When you visit a gynae you pay for a consultation and often a pelvic scan.

However, you don't get to keep any report or copy of the scan. Also no written copy of the consultation/advice or diagnosis is given after each visit. The Doctor generally maintains a file for his/her perusal only. The medicine prescription is also mostly for purposes of only the pharmacy in the clinic/ hospital. It appears one cannot buy medicines from anywhere else.
Ofcourse reports of medcail tests, if any, are generally provided to the patient.

Is this the normal procedure here?
For one's own personal record or in case one wants to take a second opinion of another doctor, a personal file with all the diagnosis, suggested treatment and scan reports would ideally be required.

Look forward to some of you shedding light on this issue.

As far as prescription medication goes, my experience in Sg is that the doctors office has it in stock so you don't have to go get a prescription filled at an offsite pharmacy as you would in some other countries. Now, I'm not 100% sure if this is true all the time for every medication. But I've found it to be the case for things such as antibiotics.

I'm not sure about the medical records question you asked, but I would think that if you asked for a copy of your records that they'd provide you one (afterall, they're your records). You might have to pay per page for photocopies or pay the mailing expenses if they're sending your info to another doctor, but that shouldn't be a big deal.

Maidsrch - Thanks for the information. Happy to hear about your successful pregnancy!! Right now I am comfortable with Dr. Anupriya. I have had a couple of visits with her. She seems good, is easy to talk to and is a PCOS sufferer hereself! Next cycle my follicular study begins. Fingers crossed. I will keep Dr Selena in mind. Many thanks

I can't comment on the drs mentioned but I have a history of PCOS, Endo and cervical cancer and I find Brenda Low to be pretty good. Nice and proactive but not all hysterical. But I am not looking to get pregnant now so I can't comment on that.

I have two children, the youngest born after all diagnoses and treatments. I know it's a tough time. Good luck with it.
