EP Collection - Supporting documents

I got IPA already, MoM also request a list of document that I need to provide during the collection of EP below..

- Passport
- Declaration form with photo sign by me
- Declaration form sign and stamp by employer

They don't mention about other documents.. Just wondering if I need to bring the medical exam result or the original copy of my education certificate to show them since they are not listed in IPA.

When in doubt take them along. I mean, how much trouble is it? You only need what they ask for but it's not like you need a prime Mover to take them is it?

The reason I asked because I don't want to have medical examination if it's not required.

I read in several topics and find out that many people need medical examination.. not sure if it's stated in the supporting document.. or everyone needs to do it..?

Not everyone needs to do the medical check-up; some are only required to sign the health declaration. The requirement is clearly stated in your IPA.
