Form EP152 for my wife

Just got approved for Singapore PR together with my wife and kids.
Would like to check that there's a Form EP 152 to be filed in by employer at the time of application.
But my wife is working outside Singapore now.
If she still need to get her employer to sign this form?

EP152 is usually for the main applicant? But to complete formalities all of you are needed.
