Does my child need Form EP 152 when doing PR application formalities

I, myself, am SG PR holder. I applied SG PR for my new born child and now got approved. When applying, an Employment Letter from my company is required. My question is when doing formalities, do we need to present a signed Form EP 152?

The approval letter will have list of documents needed. If it does not mention about that, then itis not needed.
