You're going to have to provide a link to that "additional employment request" form. I don't think it exists. It may well be that it's a closed door arrangement between the MOM and the National Universities here. Similar to the ability for Malaysian degree holders being able to obtain EP with starting salaries of $2400~2600/mo starting salaries (Not S pass), even though the stated minimum salary for a bottom tier EP is $3,300/mo. Good luck in finding that on the MOM site (it's not there, nor is it publicized). I also know that for fact as 6 months before I retired this past March I hired two Entomologists fresh Grads from Universiti Sains Malaysia on EP's with $2,600 starting salaries.
A thorough search of our archives could also turn up some info on it as I vaguely remember something of that sort cropping up a number of years ago.
I have already submitted the form and the request was denied by the MOM. My initial request to this group was to get some recommendation for the appeal. What makes successful appeal in employment cases? You have the form, if, as with most government forms and specifically MOM forms, they ask for what they want. They don't need any additional information and if they do, they will definitely get back to you to request specifics. So, get your potential employer and existing employer to fill out the necessary on the form and submit. Like I said, MOM does do some stuff that is impossible to find on the website and often the front line clerks and phone drones don't even know about. I do remember the earlier case but the details I cannot recall nor how long ago. Good Luck.
