What Happened to Annex A to Form 4A?

Hi everyone,

I was wondering what happened to Annex A to Form 4A?
I downloaded the new electronic PR form (which is great btw)
It's 13 pages long and very user friendly.

However, I noticed that there's no Annex A; which used to be standard practice in the past.
Is it now not mandatory?

It was not mandatory earlier too. If the employer refuses the fill, the employee can inform ICA on the same via a simple letter.
