EP pass application

Hi.

I'd got offer in Singapore and the company want to apply EP pass for me. Previously, I worked in 2 different companies under work permit. Since this is my first time under EP, I would like to know how the application process.

1) Does it take a long time to approve?
2) How to fill up the application form? Does the employer fill up for us? Or the employer ask us to fill up the form first before passing back to employer?
3) Do I need to go to Singapore when the company start to apply? I'm currently in Malaysia.
4) In the form got previous employment history section ( date of start & date of end) , can we just write the month and the year instead of this format dd/mm/yy.
For my previous job, the company begin my application on April however I started working on May. In the form,can I write the month of April as my date of start instead of May?

If you do not give them what they ask for, when they are vetting your application, if they find discrepancies, what do you think they will do? :-k
