How to apply for sole-proprietorship

I am trying to apply for sole-proprietorship. I have completed the application form and paid up on ACRA. I have also completed the LOC form. Although I can't find anything on the ACRA page, I was told that I would need to send an additional letter to confirm the following:

a. Detailed description of job duties
b. Venue where job duties will be performed
c. Reasons why EntrePass is not applicable
d. A statement that I will terminate all workings of the business
should your DP be cancelled.
e. A statement that I will be employing myself

Does anyone with experience of this mind sharing with me, what worked for them. Any other information that you think would be useful to me would be greatly appreciated.
