confusing paperwork from CDP

Hey all,

Help from anyone who's been through this process would be appreciated!

I'm opening, simultaneously, a CDP account and an account with a stock brokerage in Singapore (these are already linked).

I've already received my brokerage log-in info and my CDP log-in info by post. But today I got a weird one:

It's a "CDP Phone and CDP Internet Service Application" form. One of those small perforated envelopes that banks use to send you a new PIN code. There's a few fields to fill out but absolutely no indication of what it's for (they've already sent me the log-in info) or what I should do with it (e.g. where to send it or return it).

Does anyone know what this is for?

It's the Central Depository linked to the SGX (the local stock market).

It provides clearing, settlement, depository services for equities traded on SGX.

You really need to physically travel down to SGX and ask for help directly.

If not, please approach any securities department of any local bank and they would be happy to help you.
