The Challenger shuttle got brought down by a faulty O-ring, made by a company where employees were afraid to report known short-cuts.

The US President's car, 'The Beast', designed to protect against any reasonable known threat, was humbled by a speed-bump at the Dublin Embassy.

Ha ha!! Good one. It would have been comedy if the driver got his jacket trapped in the door too!

Why do they bother to ship that thing over. They should pop down and rent an old transit from Budget.....

IIRC it was reported but ignored. The guy who reported it left the company (also for that reason) few years before the disaster happened.
