OP does not mention the educational qualifications, they are cracking down on those also. Not only diploma mills, but lower quality regional universities.

I highly doubt this will be an issue - people lose their passports all the time. I wouldn't even include the police report with my application. Why confuse the MOM person reviewing your application with documents they didn't ask for? That is likely to subject it to further scrutiny than it would get...

It's not just the "regional" people who do this. For those busy running a business there's often a bit of the "leave it to the accountants" mentality even if it is more costly and inefficient in the end.

I know people who have done this but they have not had a 100% success rate. The chances of approval are highly dependent on the person conducting the visa interview.

Surname is first on both my pass, and my wife's pass.

MOM asked for the name as in passport, and on my passport (US passport) the surname comes first, so that's what they got.

I really doubt it matters to anyone considering the wide variance in naming practices in these parts..
