I've always used 2nd cousins but I'm pretty sure that is wrong and there should be a x removed in there somewhere. I have "cousins" who are the great-great grandchildren of my aunt, I've got no idea what number that should be

There is a separate health declaration form you need to fill in when visiting medical offices which is uses a different QR code and system, if you are accompanying a patient (I've done this a couple of times with my wife) then you just use Trace Together.
