I think you are still eligible for the PR application since you applied before 21 years old. I am more concern on 3 letters sent by ICA; did they send APPROVAL LETTER and because the approval letter only valid for 2 months; and you miss it. Hence, the application is withdrawn now? aged parent holding LTSVP must try to get health insurance; without it will be very expensive..

if the baby status is not PR, after 3months, either admission / hospitalization fee or outpatient check up will be charged as private..

I think only admission can use PR subsidised price, but for normal check up / test outpatient will be foreigner charge..

Currently I am paying foreigner price for my Son (3months old) and I am PR.

2 years ago, my gf (then wife) 's online SV extension was rejected also, I called ICA and they said I can go to ICA building for extension and I did that and get the extension.. May be the rule was changed.

if ur extend it online, you still can go to ICA building for extension; unless you went there and it was rejected.. you can try to go to Thailand in week end and come again; some people did success to get 1 more month

My auntie has his 1st time passport and came to singapore without any hustle from ICA;

as long your mother in law bring enough cash (just in case).

once she is here with SV, you can apply her LTSVP online; she can also extended under SV for 89 days

I got my PR 7 years ago; there was a letter to say only 2nd generation PR will need to go to NS; I think I was under PTS scheme :) exempted

saving 15mil in Singapore bank, i think it's peanut to invest 2.5M in Singapore; if I am you. I will definitely invest it;
However, from your explanation it seem this is a case of "marriage of convenience.";

why do you need PR if you do not want contribute to Singapore?
